****READ ME********I, father, mother, or guardian of the child named above, do hereby give permission to said son/daughter to participate in the Harvey Krupnick\'s Batting School activities. I hereby covenant and agree with any and all related parties of the Harvey Krupnick\'s Batting School to hold harmless without limitations all claims against personal injury, liability, or costs of any damages or expenses which may arise in connection with out son/daughter involvement in the batting school\'s program. I do hereby further testify as to my son\'s/daughter\'s sound health of mind and body. I intend this statement to take effect as a sealed instrument.

BATTING SCHOOL FEES: $295

After submitting by email your registration information please forward by regular mail your $25 enrollment fee (check) made out to North Attleboro Summer Baseball Camp and send to North Attleboro Summer Baseball Camp P.O. Box 6256, Holliston, MA 01746 and balance of $270.00 may be paid on or before the first day of camp on June 28th.
